Attendance will be limited to 450 persons, with preference given to authors; we expect the number of applications will exceed the number of spaces. Attendance is limited to current residents of the Americas. An application is required from all prospective participants, including authors. Applications will be accepted until midnight (PDT) June 4, 2004. After June 4th, organizers may be reached by directly contacting the Crowne Plaza Hotel (843-842-2400) and asking for the Solid State Sensors, Actuators and Microsystems Workshop registration desk. An Application to Attend must still be completed through this website. Cancellation policy: All refunds must have been requested in writing prior to May 24th. No refunds will be honored after that date. Due to high demand, limited space at the Workshop, and the hotel reservation deadline, attendance applications must be submitted as soon as possible. Notifications of acceptance will be mailed out periodically. Once your application form has been processed and reviewed, a notice of your application status will be confirmed to you by e-mail. If your application has been accepted, you will receive a registration form and confirmation letter. Registration payment (U.S. Dollars only) is due upon submission of registration. The fee includes program material, (1) technical digest and CD ROM, continental breakfasts, refreshment breaks, luncheons, banquet and a 20% non-refundable cancellation fee. All requests for cancellations are to be made in writing and are subject to a 20% cancellation fee. No refunds will be processed for cancellations made after May 24, 2004. There is $15 discount for payments paid by check.
